There is a $50 per person one time enrollment fee. Maximum Family Enrollment fee is $200.

The Monthly Birth to College age Membership of $25 is only valid when a Parent also has a membership.

Please note regarding the enrollment fee: This fee is non-refundable when enrolling for Direct Primary Care. This fee allows us to secure your spot in the practice and also allows the staff to obtain your past medical records while completing the onboarding process. We will contact you within 24-48 hours of your enrollment to schedule your first appointment. During the enrollment process, you will pick a date within the month for your auto draft to begin. Patients enrolling in our weight loss/HRT programs will provide a $50 Enrollment Fee online which will be applied to your start up cost. If you decide you do not want to move forward, this fee will be NOT BE refunded. We do not take medical insurance.Â

You have the option to meet with a nurse weekly for accountability for weight loss. Accountability is key for sticking with it!

We work on a membership model; however, we do not have long contracts. We only ask that you give us a 30 day notice if you want to stop your membership so we do not order anymore medication or supplies for you.

For our patients that join DPC they will receive discounted pricing on Medical Weight loss.  If you are a DPC member, your medical weight loss monthly fee will be $100 per month (in addition to your DPC age based membership rate).  You may pay for your medications in cash but we do ask that your monthly membership fee be auto drafted with a credit card, debit card, or bank account. You may also use a pre-paid debit card for the auto draft.
